Tracking your progress - the Rehabilitation and Goal Sheet
Your pain management program is a collaboration between you and your Advance practitioners. As you will be seen exclusively over telehealth it is very important to have some tools to track your progress that both you and your practitioners can see and edit “live” during your session. Your “Goal Sheet” describes the things most important in your life that you wish to return to. Your goals form the basis of our pain management program. The “Rehab Sheet” is a daily tracker of your exercise and activity progressions. An example Goal Sheet completed by a patient online is displayed below.
To enable “live” viewing and editing during your telehealth session, a link to your Goal and Rehab sheets will be emailed to you by your Advance practitioner. Simply click on this link and edit the Goal and Rehab Sheets when you need to. It’s worth keeping the original email in case you accidentally shut your browser window.
If you are not “tech savvy” its okay! Your practitioner will help guide you through the process in one of your first consultations. Before you know it you will be confident using your online Goal and Rehab Sheet!
Completing your Electronic Persistent Pain Outcomes Collaboration (ePPOC) questionnaires
So we can provide you with the best possible care, you will be asked to complete a series of online questionnaires at various stages of your program. You can do this using your phone, tablet or computer. These questionnaires allow us to be compared to all other pain management providers across Australia to help us improve our programs (all questionnaire information sent to ePPOC has your name removed).

The questionnaire is in two parts, so please make sure both parts are completed. The questionnaire will take you 45-60 minutes to complete. Take your time and answer each question as accurately as you can. If you need a break, make sure you leave your browser window open. If you close your browser window you will have to start the questionnaire again. Please complete the questionnaire at least 12 hours before your next appointment so your practitioner has access to look at it. Please note we cannot commence your pain management program until the initial assessment questionnaire is completed.
No questionnaire is perfect and there are some repeat questions. There may also times when it might seem that none of the options fit your situation. Please just pick the option you feel best answers the question for you. You need to answer all questions to complete the questionnaire. Try testing yourself during your pain management program
To further track how you are going during the program, try these simple questionnaires which you can either print, complete and discuss with your practitioner or complete online and have the scores automatically calculated. Over time with your pain management program you should see these scores improve!
